2020 Game Changer Award Nomination Form


To be eligible for consideration:

  • Nomination and supporting materials must be submitted no later than September 4, 2020.
  • Nominee must be a current corporate member of CVC-TC in good standing.
  • Nominee must not have received this award within the past three years.

Please provide overall information about the corporation being nominated.
Program Information
Please describe the program, including an overview of the collaboration between the program and its nonprofit partner.
Provide the number of Minnesota-based (or local) employees involved in the initiative/program.
Please describe the innovative aspects of this program.
Describe the outcomes of the program (short and/or long-term results), the benefits for the company and employees, and why this is significant. If you have documents that support your outcomes, please email them to info@cvctc.org.
